You opened XYZ Homework from Canvas, D2L Brightspace, Blackboard or Moodle. Instead of your assignment, or in a box titled “Error” on top of the assignment while you were working in it, you see one of these messages:
Your session has expired. Please go back to your LMS and open this assignment again.
Your session expired. Please go back to your LMS and open the assignment again.
Necessary information is missing. Please go back to your LMS and open the assignment again.
Uh oh, something went wrong. Please go back and try again. You may have launched too many assignments too quickly.
Or “Error: State not found” or “Error: Nonce was already consumed”. If you see a different message that starts with “Error:”, find it in the list of error messages on the XYZ Homework troubleshooting page, which says who can fix each one. Some of them are LMS setup problems that opening the assignment again won’t fix. Tell your instructor: only your school’s LMS administrator, sometimes with XYZ Homework support, can fix those.
What this means
XYZ Homework has lost the connection your LMS set up when you clicked the assignment. This can happen when a page sat unused for a long time, or when the page was opened again from a bookmark, your browser history, a refresh or the Back button instead of from your course. It also happens when you open a second assignment from your course and then reload, or go back to, the first one: XYZ Homework keeps track of only one assignment opened from your LMS at a time. If the message is “Error: State not found”, browser restrictions on an assignment embedded inside your course page can also be involved. Try the steps below and contact support if the message keeps returning.

Go back to your course in the LMS.

Click the assignment link there again.

Wait for the assignment to load before you open another one. Work on one assignment at a time: after you open a different assignment from your course, don’t reload or go back to the first one in its old tab or window. Open it again from its link in your course.
Don't refresh the error page
Refreshing, using the Back button, or opening a bookmark or an old tab can bring the same message back. Always start again from the assignment link in your course.
If you were partway through an assignment or test when the message appeared, open it again from your LMS. To check what was saved, see My work didn’t save, or my test says it’s in progress.
“Error: State not found”
The full message goes on to mention cookies. Go back to your course and open the assignment again. If you get the same message again, open the assignment in a new tab or window instead of inside your course page. “Error: Nonce was already consumed”
The same opening of the assignment was used twice, usually because the page was refreshed or you used the Back button. Go back to your course and click the assignment link again rather than refreshing. See Nonce was already consumed on the troubleshooting page.
Who can fix it
- You: go back to your course and open the assignment from its link, as above.
- Your instructor: if you see “Error: State not found” every time, tell your instructor. They may be able to set the assignment’s link to open in a new tab or window. (In Blackboard, only the LMS administrator can.) See XYZ Homework is blank or asks me to sign in again.
- Your college’s LMS team: fixes the “Error:” messages that the troubleshooting page lists as LMS setup problems. Your instructor can pass the message on to them.
- XYZ Homework support: if opening the assignment from your course gives you the same message every time, contact support.
If the message cost you time on a timed test or made you miss a due date, tell your instructor, and ask them about extra time or an extension.
